자주 재발하는 연령관련 황반변성에서의 신생혈관의 특징

목적 : This study was to investigate the features of neovascularization in eyes with age related macular degeneration using optical coherence tomography angiography (OCTA) according to the treatment response.
방법 : Retrospective case series.
The exudative AMD patients with type 1 neovascularization treated with ‘pro-re-nata’ regimen after three loading intravitreous anti-VEGF injection (IVI) were classified into 2 groups based on the numbers of treatment during 12 months; stable group which needed lesser than 2 injections and unstable group which needed more than 3 injections. Features of neovascularization were compared between two groups using validated software (Angiotool).
결과 : In 71 eyes, 38 and 33 eyes were classified into stable and unstable group, respectively. The unstable group had greater vessel density (1.14vs.1.56, p = 0.041), higher vessel junction point (60.32vs.86.78, p = 0.023), higher number of open ended vessels per unit area (endpoint index, 42.63vs.52.07, p = 0.004) and higher lesion inhomogeneity (lacunarity, 0.177 vs. 0.211,p = 0.022) than those of the stable group. The features associated with total IVI numbers were vessel density and endpoint index in univariate analysis(p= 0.039, p = 0.011) and endpoint index in multivariate analysis. (p= 0.005)
결론 : Features of neovascularization in eyes with exudative AMD and type 1 neovascularzation were different according to treatment response. The features of neovascularization on OCTA might be helpful for predicting the clinical outcome.
