나이관련황반변성의 드루젠성 망막색소상피박리에 대한 레이저 치료: 12개월 경과관찰 분석

목적 : To evaluate the efficacy and safety of laser photocoagulation for drusenoid pigment epithelial detachment (dPED)
방법 : A two-year study was conducted in patients with bilateral intermediate age-related macular degeneration, featuring dPED. Treatment-naïve eyes with visual acuity ≤ 83 letters were enrolled. Laser treatment group received PASCAL® laser (532 nm) along the periphery of the dPED. To prevent complications of choroidal neovascularization, intravitreal bevacizumab injections were performed on a 3-month interval up to one year. Primary outcome: drusen area, PED height, Secondary outcome: BCVA, contrast sensitivity, degree of metamorphopsia, NEI VFQ 25, fundus autofluorescence.
결과 : A total of 21 patients (mean age 72.4 years) were enrolled. This 12-month interim analysis was done in 18 patients who satisfied the follow-up period. Drusen area and PED height decreased significantly in the laser group (73.4%, 80.1% respectively, P < 0.001), while no significant change appeared in the control group. Mean BCVA improved 4.6 letters in the laser group (vs. 1.1 letters (control group), P = 0.028). As for safety, one study eye developed retinal pigment epithelial tear at month 12, and one control eye developed retinal angiomatous proliferation at month 6.
결론 : Low energy laser photocoagulation in eyes with dPED may improve functional and anatomical outcomes without serious complications. The two year results of this study will be followed, and further extension study more than 5 years is necessary to reveal the long term efficacy and safety of laser treatment for dPED.
