제 2형 당뇨 환자에서 망막전위도와 망막혈관빛간섭단층촬영으로 평가한 혈관지표와의 연관성

목적 : The aim of this study was to determine the correlation between functional (electrophysiological) and structural (microvascular) parameters in type 2 diabetes. In addition, we analyzed the ERG parameters and vascular parameters according to the diabetic retinopathy (DR) severity from early to late stages of DR.
방법 : This prospective cohort study included 366 eyes of patients with DM and 157 eyes of healthy controls. The subjects were subdivided into six groups as follows; healthy controls, subjects with diabetes and no clinically detectable DR, subjects with mild NPDR, moderate PDR, severe NPDR, and subjects with PDR. All subjects underwent full-field ERG followed by OCT and OCT angiography. The blood pressures and laboratory test data including fasting blood sugar (FBS), HbA1c, creatinine, and estimated glomerular filtration rate (eGFR) data were obtained.
결과 : There was a positive correlation between the scotopic b-wave amplitude and the superficial vessel density(VD) and negative correlation between the scotopic b-wave implicit time and the superficial VD. Also, there was positive correlation between the combined b-wave amplitude and the superficial VD and negative correlation between the combined b-wave implicit time and the superficial VD. There were no correlations between photopic b-wave, oscillatory potential and 30-Hz flicker and superficial VD. Interestingly, there were no correlations between electrophysiological parameters and deep VD.
결론 : In conclusion, we found that in diabetic patients, functional and structural impairments precede the clinically visible DR. We also found that these neural impairments evaluated by ERG well correlated with the superficial VD. As the association between vascular and electrophysiologic parameters, we speculate that our finding could indicate a link between vascular and neurodegenerative changes in DR patents.
