아메드 밸브 삽입 후 초기 녹내장 약물 치료가 수술 결과에 미치는 영향 : 방수억제제와 프로스타글란딘제제의 비교

목적 : To compare the effects of aqueous suppressants and prostaglandin (PG) analogues during hypertensive phase (HP) on intraocular pressure (IOP) and surgical outcomes after implantation of Ahmed glaucoma valve (AGV).
방법 : Data were collected retrospectively from patients who underwent HP after AGV implantation with at least 1 year follow up. HP was defined as an IOP > 21mmHg during the first 3 postoperative months. Initial topical medications during hypertensive phase were aqueous suppressants (timolol -dorzolamide fixed combination or brimonidine -timolol fixed combination) or PG analogues (latanoprost or travoprost). Primary outcome measures were IOP changes, the number of postoperative medications, and success rate. Overall success was defined as IOP ≤ 21 with a maximum of two IOP lowering medications.
결과 : Short-term reduction in IOP following treatment with initial topical glaucoma medication was larger in the aqueous suppressant group than the PG group (P=0.016). In the PG group, the number of postoperative medications was higher than the aqueous suppressant group from postoperative 3 months to 3 years (All P<0.05). The cumulative probability of failure was 20.0 % in aqueous suppressant group and 69.2% in PG group (log rank test, P < 0.001). In the logistic regression analysis, the initial use of PG analogues was significantly associated with surgical failure (P<0.001).
결론 : Aqueous suppressants as the first medication during HP was associated with better IOP control and a higher success rate than PG analogues after AGV implantation. Initial glaucoma medication after AGV implantation could affect short- and long-term surgical outcomes.
