정상안압녹내장환자와 정상인에서 콘택트렌즈 센서로 측정한 24시간 안압 변동 패턴 비교 연구

목적 : To investigate the 24-hour nyctohemeral intraocular pressure (IOP)-related patterns with contact lens sensors (CLS) in eyes with primary open-angle glaucoma (POAG) with normal baseline IOP (i.e., normal-tension glaucoma, NTG) and healthy controls.
방법 : Thirty (30) eyes of 30 NTG patients, who had wash-out period for their IOP lowering treatment, and 20 eyes of 20 healthy volunteers (IOP matched) were prospectively enrolled for 24-hour CLS (SENSIMED Triggerfish; Sensimed AG, Lausanne, Switzerland) measurement. IOP-related patterns, including mean and standard deviation (SD) of the measurements, amplitude of cosine-fit curve, acrophase (signal peak) and bathyphase (signal trough) values (mVEq), were compared between the groups. Logistic regression model was used to find factors associated with the risk of NTG.
결과 : The mean (156.0±124.8 vs. 86.3±99.8 mVEq, P=0.034) and SD (112.5±26.9 vs. 84.6±29.2 mVEq, P=0.001) of 24-hour CLS measurements were significantly greater in NTG eyes than healthy controls. The amplitude of cosine-fit curve (156.1±40.7 vs. 116.7±41.7 mVEq, P=0.002) and acrophase values (289.2±133.5 vs. 187.6±122.1 mVEq, P=0.008) were significantly greater in NTG eyes, and this phenomenon was more remarkable during sleep time. After controlling for confounding factors, the mean of CLS measurements during sleep time was significantly associated with the risk of NTG (OR=1.01, P=0.019).
결론 : Continuous monitoring of 24-hour IOP with CLS may be useful in assessing glaucoma risk, especially for POAG patients that appear to have IOP in the normal range. IOP during sleep time may be related with the development of glaucoma in these subjects.
