시신경유두 출혈을 보이는 환자들에서 브루크막개방 최소시신경 유두테폭과 유두주위 망막신경섬유층 두께의 진행속도 비교

목적 : To investigate and compare the longitudinal rate of change of Bruch’s membrane opening minimum rim width (BMO-MRW) and peripapillary retinal nerve fiber layer (RNFL) thickness in patients showing optic disc hemorrhage (DH). We aimed to see which parameter is more advantageous to detect progression earlier in these subjects who are prone to glaucomatous progression.
방법 : In this prospective cohort study, 82 subjects (82 eyes) showing optic disc hemorrhage and who had performed more than 5 times of reliable spectral-domain optical coherence tomography (OCT) tests were included. BMO-MRW and RNFL were measured with OCT at 3 month intervals. Rates of change in global and each Garway-Heath sector were calculated with linear mixed-effects model after adjusting for age, sex, and BMO area.
결과 : Mean follow-up period was 21.6±7.9 months with mean number of 7.9±2.4 OCT tests. Baseline demographics are as follows: age,58.4±10.7years; female,46.3%; intraocular pressure,14.8±3.2mmHg; mean deviation, -4.4±5.0dB. Global rate of change of BMO-MRW was -3.51±0.68um/yr and RNFL was -1.40±0.21um/yr. Rate of change was the largest in inferotemporal sector. The rate of change was significantly larger in BMO-MRW than RNFL in all sectors except for nasal sector (p<0.05). Percent reduction was significantly larger in BMO-MRW than RNFL in inferotemporal and superotemporal sectors (p <0.05).
결론 : BMO-MRW showed significantly larger rate of change than RNFL, especially in inferotemporal and superotemporal sectors in percent reduction in patients showing DH. It may be more beneficial to detect progression earlier in BMO-MRW than RNFL in eyes showing DH which are more likely to progress.
