정상 및 녹내장 눈에서 빛간섭단층혈관촬영술을 이용하여 측정한 황반부 혈관 밀도에 영향을 주는 요소

목적 : To investigate factors associated with macular vessel density (mVD) and analyze their effects according to glaucoma stage.
방법 : Design: Cross-sectional study
Setting: Glaucoma clinic at Severance Hospital of Yonsei University.
Study Population: Fifty-six healthy participants and 147 open-angle glaucoma patients.
Observation Procedures: All eyes underwent imaging using spectral-domain Optical Coherence tomography (OCT), OCT angiography (OCTA) and visual field exam.
Main outcome Measures: The relationship between mVD measured by OCTA and various variables such as age, sex, ganglion cell-inner plexiform layer (GCIPL) thickness, mean deviation (MD), and signal strength of OCTA were analyzed by linear regression.

결과 : Age and signal strength were associated with mVD in healthy eyes (P=.015 and P<.001, respectively). In glaucomatous eyes, age, signal strength, GCIPL thickness, and MD correlated with mVD (P<.001, P<.001, P=.005, and P<.001, respectively). mVD had stronger correlation with MD (R2=0.240) than with GCIPL thickness (R2=0.153) in glaucomatous eyes. Age and signal strength were correlated with mVD in early (P=.017 and P<.001, respectively) and moderate glaucoma (P=.002 and P<.001, respectively). In advanced glaucoma, GCIPL thickness (P<.001) and signal strength (P=.003) were associated with mVD.
결론 : Signal strength was the most significant factor associated with mVD in both healthy and glaucomatous eyes. mVD of glaucomatous eyes was associated with glaucomatous damage, especially functional than structural. Age was correlated with mVD in eyes with early and moderate glaucoma, otherwise, GCIPL thickness was correlated with mVD in eyes with advanced glaucoma. These results should be considered when interpreting mVD in healthy and glaucomatous eyes.
