간헐외사시 환자에서의 조절눈모음비 측정: 부가렌즈 도수 및 측정 방법에 따른 차이

목적 : To determine if there are differences between the clinical gradient stimulus AC/A ratios when measured using plus and minus lenses at distance and near fixation in patients with intermittent exotropia.
방법 : The gradient AC/A ratio was measured separately with -2.00 and +3.00 D lenses at 33 cm and with -2.00 D lens at 6 meter using an accommodative target. The angle of deviation was measured using the prism alternating cover test at near and distance. Extreme care was exercised to ensure that accommodation was maintained at a constant level throughout the whole process of testing. We additionally investigated differences in AC/A ratio using different types of fixation targets: Lang fixation stick and near vision chart.
결과 : Sixty-five patients with intermittent exotropia were examined. Their median age was 8 years (range, 4-25 years) and their median angle of deviation was 25 pd (range, 14-50 pd). Mean near stimulus AC/A ratio measured using -2.00 and +3.00 D lenses was 1.32 and 0.98, respectively (p=0.01), while mean distance stimulus AC/A ratio measured using -2.00 D lens was 1.29, which was significantly higher than that measured using plus lenses (p=0.03). The gradient AC/A ratio was found to be significantly higher when measured with a vision chart compared with Lang fixation target (p=0.01).
결론 : Significant differences were found in the stimulus AC/A ratio depending on measurement technique and accommodative stimuli. Minus lenses can be effectively used in measuring gradient AC/A ratio in patients with intermittent exotropia by driving pertinent accommodation for a given visual demand.
