코눈물관막힘 환자에서 눈물주머니코안연결술 전과 후의 기질단백분해효소 9 현장면역검사법 결과 변화

목적 : To compare matrix metalloproteinase-9 (MMP-9) point-of-care immunoassay (InflammatDry) results before and after dacryocystorhinostomy in patients with nasolcarimal duct obstruction.
방법 : 38 eyes of 28 patients who were diagnosed with nasolacrimal duct obstruction which was confirmed with dacryocystography(DCG).Patients were treated with dacryocystorhinostomy. The InflammaDry test was used to grade MMP-9 levels in tear film. Treatment response was monitored using InflammaDry test, Tear meniscus height on slit lamp, and tear meniscus parameters(tear meniscus height, depth, and area) measured by anterior segment optical coherence tomography.
결과 : Among the subjects, the number of patients with bilateral nasolacrimal duct obstruction was 10, and 18 patients had an unilateral nasolacrimal duct obstruction. Also, we did MMP-9 test on patients' both eyes (36 eyes of 18 patients) who had an unilateral nasolacrimal duct obstruction. Positive rate of MMP-9 on diseased eye was 100%(18 of 18 eyes), which was higher than positive rate of MMP-9 on healthy fellow eye(27.8%, 5 of 18 eyes) (P< 0.001, chi-square test). All patients showed improvement of tear pooling symptom. Of the eyes, 14 (36.9%) trace-positive, 8 (21.1%) weak-positive, 5 (13.1%) positive and 11 (28.9%) strong-positive for MMP-9 before the operation.Of the eyes, 10 (36.9%) trace-positive, 2 (21.1%) weak-positive, 0 (13.1%) positive and 0 (28.9%) strong-positive for MMP-9 after the operationMMP-9-positive patients displayed decreased postop MMP-9 levels (P< 0.001, Wilcoxon singed rank test).
결론 : In our study, we were able to identify that tear pooling result from nasolcarimal duct obstruction cause inflammation on ocular surface with MMP-9 test. Those patients with tear pooling symptom caused by nasolacrimal duct obstruction can relieve inflammatory reaction of ocular surface by relieving tear pooling symptom through dacryocystorhinostomy.
