아메드장치 삽입술 후 전방내 실리콘관 길이조정술이 각막내피세포 손상과 녹내장 진행에 미치는 영향

목적 : Ahmed glaucoma valve (AGV) implantation is the main surgical procedure that can be done for control of intractably elevated intraocular pressure. In this study, we intended to evaluate the effect of this postoperative trimming of Ahmed glaucoma valve on the corneal endothelial cell loss and glaucoma progression. 방법 : We prospectively evaluated the change of corneal endothelial cell count (ECC) in patients who underwent AGV implantation surgery and tube trimming surgery to treat intractable IOP elevation. IOP was measured by Goldmann applanation tonometry. A specular microscopic examination of corneal endothelial cells was performed using a non-contact specular microscope (Topcon). We compared ECC and decrease the rate of ECC in the patient who underwent tip trimming surgery with those who not underwent after AGV implantation. 결과 : A total of 70 eyes of 66 patients were included. 32 eyes received tip trimming surgery, 38 did not. The mean follow-up was 4.57 years. In the tip trimming group, the average decrease rate in corneal ECC was 12.47%/y before tip trimming surgery, whereas the decrease rate of the corneal ECC was improved by 5.52%/y after tip trimming surgery (p<0.01). The average decrease rate of the corneal ECC was 3.70%/yr in not underwent the trimming surgery group. There was no significant difference in IOP or visual field change before and after trimming. 결론 : AGV implantation surgery is effective in IIOP, and tip trimming surgery could protective in the corneal endothelial loss.
