본문 : Purpose: To report a case of unilateral peripapillary racemose hemangioma with tilted disc syndrome.
Case summary: A 19-year-old female patient was referred to our clinic due to floaters in her right eye. Her visual acuity was 20/20 in both eyes, anterior segment examination showed normal findings. Right fundus examination revealed dilated and tortuous peripapillary retinal vessels and optic disc hemorrhage. The optic disc was tilted, oval-shaped, inferonasal disc is posteriorly displaced, situs inversus, and a tessellated nasal side, which was consistent with the findings of Tilted disc syndrome(TDS). And the fundus finding of the left eye showed normal. Fluorescein angiogram of the right eye showed arteriovenous anastomosis of peripapillary retinal vessels and rapid filling of vessels with no associated leakage. Frequency doubling technology(FDT) perimetry showed temporal visual field defect aligned along the vertical meridian in her right eye. Based on the clinical findings, we made a diagnosis of unilateral peripapillary racemose hemangioma with tilted disc syndrome.
The patient was being monitored periodically, and one month after the initial visit, peripapillary disc hemorrhage was disappeared.
Conclusion: To our knowledge, this is the first report showing coexistence of unilateral peripapillary racemose hemangioma and TDS in the English literature. Because peripapillary racemose hemangioma could accompany hemorrhage of various type, a routine periodic ophthalmic examination is recommended. |
