본문 : We report two cases of secondary nasolacrimal duct obstruction after facial surgery in young patients. Case 1 was a 22-year-old woman, who presented with left-sided tearing in the eye since one year. Lacrimal syringing test showed reflux of saline, and dacryoscintigraphy revealed a definite delay in tear flow in the left eye. One year ago, she had undergone Le Fort II osteotomy for correction of malocclusion. In a computed tomography (CT) scan, narrowing of the nasolacrimal duct by the fractured fragment of bone of the lacrimal fossa was evident. After endoscopic dacryocystorhinostomy and removal of the bone fragment, symptoms of tearing abated. Case 2 was a 19-year-old man, who presented with left-sided tearing in the eye since six months. Lacrimal syringing test showed partial reflux, and dacryoscintigraphy revealed a definite delay in tear flow in the left eye. One year ago, he had undergone a surgery for reconstruction of inferior orbital wall fracture. On CT scan, inward displacement toward the lacrimal fossa of the implant placed in inferior wall of orbit was evident. After removal of the implant and endoscopic dacryocystorhinostomy, symptoms of tearing abated. |
