목적 : The purpose of this study was to determine the factors associated with the recurrence of infection after therapeutic penetrating keratoplasty (TPKP).
방법 : Retrospective medical chart review of patients who had been diagnosed with medically uncontrolled infectious keratitis and received TPKP at Seoul National University Hospital between January 1st, 1995 and December 31st, 2018, was done. General demographic information including medical history, slit lamp microscopic findings and microbial results from corneal scraping culture were obtained. Patients were analyzed according to rate of recurrence of infectious keratitis after first TPKP. Patients under 18 years of age or with endophthalmitis at presentation before first TPKP were excluded.
결과 : Among 21 patients, 8(38.1%) patients showed recurrence of either same(25%) or different microorganism(75%).17% of patients, who received primary PKP for other reasons, showed recurrence. Male sex(p=0.020), pseudophakia(p=0.044) and low intraocular pressure(p=0.023) at presentation and TPKP within 30 days(p=0.026) since symptoms or signs were seen more in no recurrence group compared to recurrence group. TPKP between 16 to 30 days showed lower recurrence than when done after 30 days(p=0.005) and within 15 days(p=0.051).Evisceration was done in 14.3% of all patients and 37.5% of recurrence group
결론 : 38.1% of all patients who received TPKP resulted in recurrence after first TPKP and 50% of patients who already had their first recurrence experienced repeated recurrences of infection afterwards despite a second TPKP. The female sex was significantly higher in the recurrence group while pseudophakic eyes and decreased intraocular pressure at presentation were significantly greater in patients with no recurrence. First TPKP done within 30 days or between 16 to 30 days had lower recurrence rates.
