Successful Anticoagulant Treatment of Non-arteritic Ischemic Optic Neuropathy (NAION) with Thrombosis

Purpose : To present a case of Non-arteritic Ischemic Optic Neuropathy (NAION) with thrombosis treated successfully with anticoagulant Methods : A 47-year-old women admitted to hospital with blurred vision on the left eye since 3 weeks ago. Ophthlamologic examination revealed visual acuity of 20/20 and 0.5/60 on her right and left eye, respectively. Positive RAPD was found on the left eye. Fundus examination showed a hyperemic papil with blurred margin, C/D ratio cannot be measured on the left eye. Small C/D ratio was seen on her right eye. Visual field examination showed an inferior arcuate visual field defect on her left eye. On laboratory examination, D-dimer was extremely high (9210 ug/L) and fibrinogen also increased (680.4 mg/dL). This patient treated with anticoagulant for 1 month. Results : Laboratory examination post anticoagulant treatment was obtained on her next visit, showed a significant decrease of D-dimer (1.450 mg/dL). Improvement of visual acuity and visual field was seen in this patient. Conclusion : Treatment of trombosis may have benefit in NAION patients.
