Anti-Aldolase C 자가항체가 검출된 한국인 흑색종 관련 망막병증 증례 1예

본문 : Purpose: To report a first case in Korea of melanoma-associated retinopathy. Case summary: A 58-year-old man with cutaneous melanoma presented with decreased vision. Best-corrected visual acuity at presentation was 20/32 on the right eye and 20/2000 on the left eye with unremarkable refractive error. Slit lamp examination showed 1+ inflammatory cells in the anterior vitreous in both eyes. Fundus photography showed a juxtafoveolar hypopigmented chorioretinal lesion in the left eye and fluorescein angiography of the left eye revealed late leakage at the same lesion consistent with occult choroidal neovascularization. Spectral domain optical coherence tomography revealed unremarkable finding in the right eye and focal pigment epithelial detachment without subretinal fluid collection in the left eye. 30-2 Humphrey visual field test showed constricted tunnel vision on the right eye and total defect in the left eye. The full-field ERG showed an electronegative waveform in both eyes. Antibodies to aldolase C was identified in serum sample from the patient. All the clinical and immunologic features were suitable for a diagnosis of melanoma-associated retinopathy. Conclusion: This case report is the first to report a typical melanoma-associated retinopathy patient in Korea.
