각막윤부 유피종의 수술 후 결과의 예측 인자로서 병변부 기하학적 지표 분석의 유용성

목적 : To search for novel geometric parameters of corneal limbal dermoids that enable the prediction of clinical and surgical outcomes. 방법 : Patients with corneal limbal dermoids (85 eyes of 85 patients) were assigned to the following 4 cohorts: the postoperative visual acuity (cohort 1, n=65); amblyopia (cohort 2, n=67); the postoperative scar formation (cohort 3, n=51); the preoperative progression of dermoids (cohort 4, n=39). For geometric profiling of dermoids, 11 geometric parameters of a lesion and 9 ratio parameters were preoperatively defined using digital photographs. Geometric parameters and demographic variables were analyzed for correlation with postoperative outcome and preoperative spontaneous dermoid progression. 결과 : The mean patient age was 5.98±0.68 years at surgery. Amongst geometric and demographic parameters analyzed, the ratio of invaded angular axis of dermoid to corneal diameter-length (IAD/C-LR) had the highest r coefficient (r=0.678) in cohort 1 and best stratified the patients in cohort 2 (cut-off>0.4841) and in cohort 3 (cut-off>0.5646). No parameters were correlated with spontaneous dermoid progression in cohort 4. 결론 : We devised geometric parameters to evaluate corneal limbal dermoids before surgery and predict the surgical outcome. The value of IAD/C-LR was the most significant factor associated with the postoperative VA, amblyopia development, and postoperative scarring.
