젊은 한국인 정상안에서 시신경유두주위 혈관밀도와 관련된 요인 분석

목적 : To investigate factors (perfusion pressure vs. peripapillary structural parameters) that are more related to peripapillary vessel density (pVD) as measured by spectral-domain optical coherence tomography angiography (SD-OCTA) in young healthy Koreans. 방법 : Healthy Korean participants (age < 45 years, n = 258) were prospectively recruited. Eyes underwent 3 x 3 mm optic disc SD-OCTA scan to acquire pVD (total area of perfused vasculature per unit area in a region of measurement) in radial peripapillary capillary (RPC) layer. Optic nerve head (ONH) parameters (including retinal nerve fiber layer thickness [RNFLT], rim area, and disc area), systemic blood pressure (BP), intraocular pressure (IOP), and mean ocular perfusion pressure (MOPP) were measured. Linear mixed model was used to investigate the systemic and ocular factors associated with pVD. 결과 : The pVD showed a positive correlation with RNFLT (r = 0.224, P < 0.001) but with marginal significance with MOPP (r = 0.107, P = 0.08). In the sectoral analysis, RNFLT revealed significant positive correlation in all quadrants (all Ps< 0.05) whereas MOPP showed significance only in the temporal quadrant (r = 0.158, P = 0.010). In the multivariable analysis, male gender (P=0.016), thinner RNFLT (P = 0.004), smaller rim area (P < 0.001), greater disc area (P < 0.001), and weaker signal strength (P < 0.001) were significantly associated with decreased pVD. 결론 : The pVD in RPC layer had stronger associations with ONH parameters rather than estimated MOPP. SD-OCTA imaging may not fully capture the perfusion pressure change but is rather more dependent on peripapillary structure. Therefore, pVD shall be interpreted qualitatively as characterizing metabolically active structures rather than quantifying the blood flow.
