본문 : Purpose: To report a surgical technique for correcting residual small vertical deviations after ophthalmic surgery.
Methods: This case series included 3 patients who underwent a graded partial inferior rectus (IR) tenotomy in the contralateral eye to correct a residual hypertropia and excyclotorsion after inferior oblique recession or scleral buckling.
Results: Preoperatively, all 3 patients had an incomitant hypertropia of 6-10 PD in primary position with 2-5 degrees of excyclotorsion. Two patients underwent a graded IR marginal tenotomy (70-80%) using topical anesthesia and 1 patient had a central IR tenotomy (4 mm) and posterior fixation performed using general anesthesia. Postoperatively, all patients were orthotropic in all positions of gaze without excyclotorsion.
Conclusion: Graded partial IR tenotomy is an effective surgical option to correct small residual vertical deviations and excyclotorsion as a secondary procedure in lieu of wearing spectacles with a prismatic correction. |
