Orbital abscess secondary to rhino-sinusitis in a healthy six-month-old infant

Purpose : To report a rare case of orbital abscess as a complication of sinusitis in a healthy infant Methods : case report Results : A six-month-old healthy term baby boy, presented to emergency department with one day history of left eye redness and swelling. It was associated with fever and runny nose. Upon systemic examination, the child was active with low grade fever, other systemic examination was unremarkable. His ocular examination showed left tender periorbital swelling with injected and chemosed conjunctiva. There was no relative afferent pupillary defect and horizontal ocular movement was normal. Examination of his left cornea, anterior chamber and fundus were normal. The fellow eye examination was normal too. His blood culture and eye swab culture grew methicillin sensitive Staphylococcus Aureus (MSSA) and he was treated with intravenous augmentin. Computed- tomography scan of his brain, orbit and paranasal sinus was performed and it showed presence of left intraorbital extraconal abscess extended to the left ethmoid and maxillary sinuses. In view of slow response to the antibiotic, left eyelid incision and drainage with nasal decongestion was done with the otorhinolaryngology team. His condition improved remarkably after the operation. Conclusion : Although orbital abscess is a rare complication of sinusitis in infants, imaging such as CT or MRI scan of orbit should be performed early to confirm clinical suspicion. Delayed in making a correct diagnosis and treatment may result in permanent visual impairment or even death to the child.
