정상안압녹내장과 비동맥염성 앞허혈 시신경병증 안의 사상판 곡률의 비교

목적 : To compare the lamina cribrosa (LC) morphology between nonarteritic anterior ischemic optic neuropathy (NAION) and normal tension glaucoma (NTG). 방법 : This study included 48 eyes with NAION, and NTG and healthy control eyes who were matched with NAION eyes in terms of age, intraocular pressure, axial length, and optic disc area. The RNFL thickness in the affected sector was also matched between NAION and NTG patients. Optic nerve heads were scanned using spectral-domain optical coherence tomography and the LC depth (LCD) and LC curvature index (LCCI) were measured at 7 locations. The LCD and LCCI were compared between 3 groups, and the diagnostic powers of those parameters in discriminating the NAION from NTG were determined. 결과 : The LCD and LCCI were significantly larger in NTG group than in the NAION and healthy control groups at all 7 planes (Ps < 0.001), while comparable between NAION and healthy control groups. The diagnostic ability of the average LCCI for discriminating NAION from NTG was excellent (AUC, 0.937; 95% confidence interval, 0.894-0.981; sensitivity, 81.3 %; specificity, 91.7 %) and better than that of average LCD (AUC, 0.772; 95% confidence interval, 0.679-0.865; sensitivity, 68.8%; specificity, 77.1 %) in DeLong’s test (P < 0.001). 결론 : Eyes with NAION had different LC morphology as compared to those with NTG despite of similar degree of RNFL damage. Evaluation of LC morphology may be beneficial to differentiate NAION from NTG eyes.
