본문 : Purpose : To report a case of ocular syphilis presenting as unilateral papillitis
Case summary : A 53-year old woman presented with floaters that had begun 1 month previously. Best corrected visual acuities were 20/20 in both eyes. Anterior segment was normal. On fundoscopic examination, optic disc swelling was found only in the left eye. Optical coherence tomography showed diffuse RNFL swelling in the left eye. Serology was positive for fluorescent treponemal antibody absorbance (FTA-ABS) IgG, treponema pallidum hemagglutination (TPHA) test, veneral disease research laboratory (VDRL) test and rapid plasma reagin (RPR) test. There was no specific evidence of infectious diseases in cerebrospinal fluid testing. A diagnosis of unilateral syphilitic optic neuritis was made after confirmation of no evident brain lesion by MR venogram, Brain MRI. The patient was treated with antibiotics with systemic steroid therapy. After 2 weeks of treatment, disc swelling improved on left fundoscopy.
Conclusions : Ocular syphilis can present as s unilateral and asymptomatic papillitis. |
