목적 : To assess clinical efficacy of sequential intrastromal corneal ring segment (ICRS) implantation and corneal crosslinking (CXL) in corneal ectasia.
방법 : This retrospective case series comprised of 8 eyes in which both intrastromal corneal ring segment (ICRS) insertion and corneal collagen crosslinking (CXL) had been applied, and the clinical outcome were compared with that in whom ICRS (8 eyes) or CXL (20 eyes) only had been applied. ICRS implantation was sequentially followed by CXL in 1 month. Visual acuities, refractive errors, keratometry, topographic indices were compared.
결과 : Improvement of uncorrected visual acuity in ICRS + CXL group was significantly greatest in both 6 (P = 0.008), 12 months (P = 0.028) compared with ICRS or CXL alone. Reduction of refractive errors in sphere and spherical equivalent was significant in ICRS (P = 0.002, P = 0.004 at 6, 12 months respectively) and ICRS + CXL group (P < 0.0001, P < 0.0001 at 6, 12 months respectively). Kmax, Kmin and Kavg were significantly reduced in all 3 groups at postoperative 6, 12 months. The amount of reduction of Kmax, Kmin and Kavg was the greatest in ICRS + CXL group compared with other groups (P<0.0001).
결론 : Sequential ICRS implantation followed by CXL in 1 month seems to be effective and superior in improving visual acuity and reducing refractive errors and keratometric values than ICRS or CXL alone.
