백내장 수술 후 발생한 양안 데스메막 분리에서 전방 내 가스주입술시 자세의 중요성

본문 : Purpose: To describe a case of bilateral Descemet’s membrane detachment (DMD) after uneventful cataract surgery and to discuss the importance of the patient’s head position after gas injection. Results: We report a case of a 57-year-old woman who developed bilateral extensive DMD after uneventful cataract surgery by two skillful surgeons separately. Several gas tamponade procedures in supine position were shown to be unsuccessful in both eyes. In contrast, extensive DMD was recovered dramatically the day after the gas tamponade at temporal incision site of right eye maintained in the left-decubitus position. Treatment outcome, both in DMD resolution and visual acuity, was better in the right eye where the decubitus position was applied than in the left eye where the supine position was applied. Conclusions: Even in uneventful cataract surgeries, surgeons should be cautious with apparently normal eyes with previous DMD history of the fellow eye. In addition, if extensive DMD is detected following cataract surgery, immediate gas tamponade with proper head position is crucial to seal off the DM break site by the gas bubble.
