A comparative study on the efficacy of medial rectus recession with Faden operation in consecutive esotropia

Purpose : To evaluate the efficacy of the Faden procedure on the correction of consecutive esotropia (ET). Methods : This retrospective study included 25 children who developed consecutive ET after primary bilateral lateral rectus recession and underwent medial rectus (MR) recession with the faden procedure (Faden group) or MR recession only (control group) between 2013 and 2018. We evaluated postoperative deviation angles at each follow-ups until postoperative 6-month visit and compared surgical motor and sensory outcomes between the Faden and control group. Results : There were 10 children in the Faden group and 15 children in the control group. While the Faden group maintained orthotropia without any small deviation until postoperative 6-month visit, the control group showed wide distribution of postoperative deviation angles corresponding to 1 patient with small angle esodeviation < 5 PD, 3 patients with esodeviation > 5 PD, and 3 patients with exodeviation < 5 PD in the control group. In the Faden group, 7 patients have good (60 arc seconds or better) stereopsis and 3 demonstrated fair (80 – 3000 arc seconds) stereopsis. In the control group, 4 showed good, 8 showed fair, and 3 showed nil stereopsis (P = 0.026). While all patients in the Faden group demonstrated distant fusion, 46.7 % of the control group demonstrated distant suppression (P = 0.020). Conclusion : MR recession combined with the Faden operation could be a good surgical option for management of consecutive ET.
