당뇨황반부종에서 유리체강내 덱사메타손 임플란트 삽입술 후 맥락막 두께 변화

목적 : To investigate changes in choroidal thickness after intravitreal dexamethasone (DEX) implant injection for the treatment of diabetic macular edema (DME). 방법 : In this retrospective interventional case-series, eighty one eyes of 70 patients with DME were first treated with the injection of DEX implant. Patients were followed up for 14 weeks without any other treatment for the evaluation of best corrected visual acuity (BCVA), subfoveal choroidal thickness (SFCT) and central macular thickness (CMT) by spectral-domain optical coherence tomography (SD-OCT). 결과 : The mean baseline SFCT (299.8±116.60μm) and CMT (473.35±163.73μm) significantly decreased throughout 14 weeks (p<0.001 for SFCT and p<0.001 for CMT, respectively). The baseline logMAR BCVA (0.609 ± 0.363) significantly improved at week 7 (p=0.002) and week 14 (p=0.028). The change in SFCT at week 7 was significantly greater in the patients with submacular fluid on OCT before DEX implant treatment (p=0.039), and also in the patients with good anatomic response with CMT less than 300μm at week 7 (p=0.030). 결론 : DEX implant significantly reduced SFCT and CMT with BCVA improvement in eyes with DME throughout 14 weeks. The SFCT at week 7 was significantly reduced in eyes showing good anatomical response at week 7 and those with submacular fluid before treatment.
