목적 : To investigate the incidence of fellow eye (FE) neovascular age-related macular degeneration (nAMD) in patients with unilateral typical neovascular age-related macular degeneration according to different drusen subtypes of FE.
방법 : Medical records of both eyes of consecutive 156 patients with unilateral typical nAMD between January 2013 and June 2016 were retrospectively evaluated. The occurrence of nAMD in the fellow eye was monitored by fundus examination and optical coherence tomography for at least 24 months. The incidence of fellow eye and hazard ratios (HR) of different drusen subtypes after correcting age, sex and choroidal thickness were analyzed.
결과 : The 5-year incidence of nAMD in fellow eye in soft plus subretinal drusenoid deposit (SDD), Soft drusen only and SDD only groups were 62.0%, 50.2%, 28.6%, respectively and significantly higher than no drusen group (vs. 1.9%). However, there was no significant difference between the pachydrusen group and no drusen group. In Cox hazard model, compared with no drusen, soft drusen plus SDD, soft drusen only, and SDD group showed HR 14.524, 11.546, and 9.503 folds significantly high risks, respectively. Pachydrusen did not show significant risk than no drusen group.
결론 : According to different drusen types of FE, the incidence of FE nAMD was significantly different. In patients with unilateral nAMD, drusen types of FE could help predicting nAMD occurrence of second eye. Unlike conventional soft drusen or SDD, pachydrusen does not seem to increase the risk of neovascular AMD.
