녹내장 의증에서 녹내장 발생을 확인을 위한 황반부 GCIPL의 진행성 변화 추적

목적 : To evaluate progressive thinning of the macular ganglion cell–inner plexiform layer (GCIPL) and peripapillary retinal nerve fiber layer (RNFL) in glaucoma suspects. 방법 : The participants were glaucoma suspects who had a glaucomatous appearance in the optic nerve head and/or ocular hypertension (IOP > 21 mmHg) without repeatable visual field (VF) defects at baseline. Progressive GCIPL and RNFL thinning was determined using Guided Progression Analysis (GPA) in spectral-domain optical coherence tomography (OCT). The development of VF defects was defined as the presence of 3 consecutive abnormal VFs. 결과 : The study included 300 eyes of 300 glaucoma suspects (mean follow-up, 5.4 years). Nineteen (6.3%) and 18 (6.0%) eyes showed progressive GCIPL and RNFL thinning, respectively, while VF defects developed in 4 eyes (1.3%). Among the 13 eyes with progressive results in both GCIPL and RNFL GPA, progressive GCIPL thinning preceded (7 eyes, 53.8%) or occurred concomitantly (4 eyes, 30.8%) with progressive RNFL thinning. Most of the progressive GCIPL and RNFL thinning (83.3% [20/24]) was found in eyes that did not develop VF defects during follow-up. 결론 : In this longitudinal observation of glaucoma suspects, progressive GCIPL thinning was frequently detected before progressive RNFL thinning or the development of VF defects. Macular OCT imaging may be a useful ancillary tool for the early detection of glaucoma development.
