Purpose : To describe the various findings in a case of self-inflicted handheld laser exposure. Methods : We evaluated multimodal imaging including fundus autofluorescence, spectral-domain optical coherence tomography (SD-OCT), OCT angiography, fluorescein angiography, and indocyanine green angiography for a 25-year-old man with decreased vision after the repetitive self-inflicted handheld laser exposure. Results : Funduscopic examination revealed unilateral and asymmetric yellow macular patchy with irregular pattern in the superior macula. Dense hyperreflectivity involving all outer retinal layers, and terminating at the outer plexiform layer were seen on SD-OCT. The patient also underwent OCT angiography (OCT-A), which in the choriocapillary segmentation revealed hypointense lesions in correspondence of the focal hyperreflectivities detected on structural OCT. Conclusion : The recognition of characteristic self-inflicted handheld laser-induced lesions is imporant as laser pointer misuse has been increasing over the years.
