목적 : This is the first report to describe early complication cases of XEN gel stent (XEN45, Allergan, CA, US) in Korean open-angle glaucoma (OAG) population.
방법 : This single-center, retrospective study included a total of 14 eyes of OAG with XEN gel stent implantation. Various complications were described including hypotony (<6mmHg), choroidal detachment, corneal wound leak, implant displacement, postoperative hyphema, intraoperative bleb hemorrhage and etc. None of the severely complicated cases were included.
결과 : Hypotony was the most common complication of XEN gel stent implantation(85.7%). Early hypotony should be managed using intracameral viscoelastic injection because prolonged hypotony could be followed by choroidal detachment resulting in unexpected vision loss. The intraocular pressure (IOP) of all hypotony cases were normalized in 3 days - 3 weeks after viscoelastic injection. One case presented an IOP spike after second viscoelastic injection which needed an I&A of residual viscoelastic in anterior chamber. There was one case of corneal wound leak, implant displacement, hyphema, respectively.
결론 : XEN gel stent implantation is minimally invasive in terms of conjunctival wound healing process and highly effective at least in early postoperative period in Korean OAG population. However, surgeons should be aware of the higher chance of hypotony rate after Xen gel stent implantation than previous studies reported from other countries.
