안구 부속기에 발생한 점막 관련 변연 B-세포 림프종과 폐암이 병발한 중복암 증례보고 1예

본문 : Purpose: We report a case of Primary marginal zone B-cell lymphoma of mucosa-associated lymphoid tissue of orbital adnexa and lung cancer that occurred at the same time. Case summary: A 74-year-old man who was diagnosed with lung cancer in 2016 and was undergoing on chemotherapy. He was referred to the outpatient clinic with conjunctival mass in the right eye for 1 week. On MRI examination, we found a mass on the right eye conjunctiva, a mass abutting optic nerve on the right eye’s retrobulbar space and a mass at the left inferior extraconal space. We double checked if there were any special conditions in orbital adnexa from contrast enhancement brain magnetic resonance imaging taken in 2016 and this MRI was taken to check the presence of brain metastasis in the process of making diagnosis of lung cancer. There was no conjunctival mass on the right eye. However, there was a mass abutting optic nerve on the right eye’s retrobulbar space and a mass at the left inferior extraconal space. This confirmed the presence of ocular adnexal masses from the time of diagnosis of lung cancer in 2016. We performed biopsy for accurate diagnosis. The diagnosis was Extranodal marginal zone B cell lymphoma of mucosa-associated lymphoid tissue in the histological examination. There was no metastasis in the systemic examination. Radiotherapy was performed on the lesions 17 times. Conclusions: Simultaneous occurrence of marginal zone B-cell lymphoma of orbital adnexa and lung cancer has not been reported. Therefore, the authors would like to inform people that the orbital adnexal lymphoma and lung cancer may be present as synchronous cancers and that they should not miss the diagnosis with the possibility of synchronous cancer.
