본문 : Herpes zoster ophthalmicus (HZO) is defined as herpes zoster involvement of the ophthalmic division of the fifth cranial nerve. Optic nerve involvement is very rare sequelae of HZO, and usually reported in patients with an immunocompromised status. We report a case of optic neuritis complicated by HZO in healthy adult.
A 43-year-old male visited our clinic for sudden onset of visual disturbance in the right eye for 2 days. He suffered HZO over right side 1 month age and was treated at other clinic. He had no history of other previous systemic disease. There was no abnormal finding at ocular check-up 6 days ago. There was skin rash over the right side of his forehead. Visual acuity was no light perception in the right eye and 20/20 in the left eye. The pupil showed afferent pupillary defect in the right eye. There was no ocular moving pain at both eyes. The anterior segment and fundus examination showed normal finding. There were no significant findings in laboratory testing. Magnetic resonance imaging of brain showed no abnormalities. He received intravenous acyclovir and steroid for 5 days. However, the visual acuity of the right eye did not improve and the optic atrophy developed after 3 months. |
