Unusual Laser Blepharoplasty complications: sight and life-threatening!

Purpose : To describe a rare complication of laser blepharoplasty Methods : Case report Results : A 53 years old lady was referred by a private aesthetic clinic with bilateral severe periorbital swelling and reduce left eye vision for 5 days. She had undergone laser blepharoplasty 1 week prior to presentation. Ophthalmic examination revealed both eye periorbital swelling with pouring pus discharge from the lower lid with underlying erythematous and ulcerative skin. Extraocular muscle movement is limited due to generalized severe conjunctival chemosis. Visual acuity for right eye was 6/9 while the left eye was counting finger(CF). Anterior segment findings of Right eye was normal, but Left eye showed significant inflammation with anterior chamber cells and flare, that lead to the poor vision. The relative afferent pupillary defect was negative and there is no sign of optic nerve compression. CT scan of orbit showed rim-enhancing pre-septal collections with air pockets bilaterally involving the lower eyelids measuring 2.6cm x 4.3cm x 2.7cm on the left and 2.3cm x 4.2cm x 2.8cm on the right with extending of collections into the lateral extra-conal region. Extraocular muscles and optic nerve preserved. The patient underwent surgery with incision and drainage of abscesses. Collected pus samples were sent for culture and sensitivity and results indicated the growth of Staphylococcus epidermidis species. She was started with eye drop ciprofloxacin, systemic Ceftriaxone, and Metronidazole. the patient was referred to surgical wound team and was started with Dermacyn dressing. She shows significant improvement after 3 weeks of treatments. Subsequent visits in clinic patient condition improved with restored of visual acuity and eye movement. Obvious scarring and hyperpigmented lids can be seen but with no risk of corneal exposure. Conclusion : Periorbital abscess is rare complications from cosmetic laser blepharoplasty, however, it can lead to devastating sight and life-threatening events if a delay in treatment.
