Candida Guiliermondii , a rare and devastating cause of red eye: A case report and literature review

Purpose : To report a rare case of orbital apex syndrome with frontal abscess due to progressive sinusitis secondary to Candida Guilliermondii and provide literature review of this rare entiti. Methods : A retrospective review of patient’s medical record was done. Patient's charts was analyzed for data including; initial eye symptoms, diagnostic investigations including imaging, primary treatment given and patient morbidity were evaluated Results : A 54-year- old lady presented with progressive blurring of vision in left eye for 3 weeks,associated with drooping of left eyelid and protrusion of eyeball. Right eye vision was 6/18 and perception of light in the left eye with positive RAPD. There was left orbital apex syndrome with ptosis, proptosis, opthalmoplegia and concurrent sinusitis. CT scan of the brain showed orbital apex syndrome of the left eye with left frontal abscess secondary to sinusitis. Nasal endoscopy was performed and revealed purulent lesion in both nasal cavities extended to nasopharynx region. Biopsy was taken revealed fungal infection of Candida Guilliermondi sp. origin. C. guilliermondii is an opportunistic organism and is part of human skin and mucosal surfaces normal flora. It has been reported to express greater multidrug resistance compare to other Candida sp especially in immunocompromised host.Candida guilliermondii causing acute invasive sinusitis with orbital apex syndrome is extremely rare and its association with cerebral abscess has never been reported. Conclusion : Candida Guilliermondii is an opportunistic pathogen that rarely the cause of invasive candidiasis in immune-compromise human. It has poor clinical outcome. A collaborative management between respective teams is important for a comprehensive management of such cases
