Iatrogenic Ocular and Periocular Complications Caused by Cosmetic Facial Filler Injections: Five Case Series in a Tertiary Referral Center

Purpose : To investigate the clinical manifestations, management, and outcome of ocular and periocular complications caused by cosmetic filler injections Methods : Retrospective review of five consecutive patients with ocular complications caused by cosmetic facial filler injections referred from primary care physicians to a tertiary referral center Results : Two cases presented with central retinal artery occlusions, two patients with ptosis and extraocular muscles limitations, and one case presented with choroid vascular occlusion. Injected fillers included hyaluronic acid (1 case), calcium hydroxylapetite (Radiesee, 2 cases), poly-L-lactic Acid (Sculptra, 1 case), and polycaprolactone with carboxymethylcellulose (Ellanse, 1 case). Injection sites were the glabellar region and nasolabial fold. One patients receive hyaluronidase injection and all five patients underwent hyperbaric oxygen therapy. Patients with central retinal artery occlusions (hyaluronic acid and calcium hydroxylapetite) had the worst visual outcome. Ptosis, impairment of ocular motility, and skin necrosis of glabellar region got improvement. Conclusion : With the increase use of various cosmetic fillers in plastic and esthetic surgery, the possibility and management of severe ocular complications should not be neglected.
