Case series of Air Pneumodescemetopexy in Repair of Descemet’s Membrane Detachment

Purpose : To study the results of using air to repair Descemet’s membrane detachment (DMD) as a complication after phacoemulsification. Methods : Patients who suffered from DMD after phacoemulsification and treated with air pneumodescemetopexy (aPDP) performed by a single surgeon at Grantham Hospital in Hong Kong between September 2016 to August 2017 were reviewed retrospectively. Primary outcome measure was anatomical success rate. Anatomical success was defined as complete reattachment of Descemet’s membrane evidenced by anterior segment optical coherence tomography. Secondary outcome measure was visual acuity (VA) 3 months after aPDP and complications from procedure. Results : A total of 5 patients suffered from DMD after phacoemulsification and received PDP. Air was used for repair of DMD and no long-acting gas was used. Four of them (80%) achieved anatomical success after one aPDP. The remaining achieved anatomical success after second aPDPs. All patients had VA improvement of at least two lines on Snellen Chart at 3 months post-operatively. All final VA was better than that before cataract operations. One patient had pupil block on Day 0 after aPDP and the intraocular pressure became normal after air release on Day 0. There were no other complications from aPDP. None required cornea transplant. Conclusion : In summary, air pneumodescemetopexy is effective to repair DMD with high success rate. Special precaution should be made to prevent pupil block with high intraocular pressure which is the major complication of aPDP.
