Comparison of clinical features of cytomegalovirus retinitis between patients underwent solid organ transplantation and bone marrow transplantation.

목적 : To compare the cause, demographic, ophthalmic presentation, serology and visual prognosis of cytomegalovirus (CMV) retinitis between patients underwent solid organ transplantation (SOT) and bone marrow transplantation (BMT). 방법 : A retrospective review of 39 patients (28 male and 11 female, 57 eyes) with CMV retinitis was performed. Best corrected visual acuity (BCVA in logMAR), serologic studies (CMV polymerase chain reaction (PCR), duration of viremia, absolute neutrophil count (ANC)) and number of ocular injection were compared between SOT (25 eyes of 18 patients, 49.00(±12.95) years old) and BMT group (33 eyes of 21 patients, 24.90(±19.76) years old). 결과 : CMV retinitis occurred 177.88(±132.59) days after transplantation surgery in SOT, while 144.76(±128.67) days after transplantation, which was not different significantly. Bilateral involvement was higher in BMT than SOT (57.14% vs. 33.33%, p<0.001), and so macular involvement (4.17% vs. 12.12%, p<0.001). Peak CMV concentration was higher in BMT than SOT (p<0.001), however duration of viremia was longer in SOT than BMT (129.11(±181.85) vs. 76.81(±48.29) days, p<0.001). More intravitreal ganciclovir injection was done on SOT group than BMT group (10.37(±14.49) vs. 2.09(±2.89), p<0.001). 결론 : Visual prognosis was generally poor in patients with CMV retinitis. Bilateral and macular involvement was higher in BMT. More intravitreal ganciclovir injection was performed on SOT group, but poorer survival rate in BMT group should be considered in comparison of visual prognosis
