Relationship of choroidal microvasculature dropout with structural vs. functional loss in glaucoma

목적 : To assess the correlation between choroidal microvasculature dropout (CMvD) versus structural/functional parameters in glaucoma 방법 : OAG eyes with single detectable CMvD located at inferior hemiretinae by OCTA images of choroidal layer were consecutively enrolled. The angle between the two lines which connect the center of optic disc and either margin of CMvD was defined as angular extent (˚) of CMvD. cpRNFLT and VFMS were measured on global and sectoral area. Correlation analyses between CMvD angular extent and structural/functional parameters were performed. Multivariate linear regression analysis was performed for cpRNFLT, mGCIPLT, and VFMS as independent variables and CMvD angular extent as dependent variable. 결과 : All 62 CMvDs were located at IT region. Globally, both structural and functional parameters had significant correlation with CMvD angular extent. R2 (linear) was 27.1% for VF MS, 11.5% for mGCIPLT, and 11.0% for cpRNFLT. VFMS of superior arcuate field had closest correlation with CMvD (R2 = 39.1% for linear). Superior arcuate and global VFMS had significant independent relationships with CMvD angular extent (standardized β=-0.62 and -0.47; P < 0.001) while adjusting for cpRNFLT and mGCIPLT. Same results were obtained by including only eyes with mild to moderate stage glaucoma (MD≥-10 dB). 결론 : In OAG eyes, CMvD had quantitative correlation with both structural and functional parameters. The coefficient of determination between CMvD angular extent and VF MS was higher than that between CMvD angular extent and structural parameters. Our findings suggest that increase in CMvD size may be better reflected in functional loss than structural loss in glaucoma.
