Dynamic range of peripapillary retinal vessel density to detect glaucomatous visual field damage in comparison with circumpapillary retinal nerve fiber layer thickness

목적 : To determine the dynamic range of peripapillary retinal vessel density (ppVD) to predict the severity of visual field (VF) damage in comparison with that of circumpapillary retinal nerve fiber layer thickness (cpRNFLT) in primary open-angle glaucoma (POAG) patients, and to determine the level of VF damage below which progressive VF loss becomes not detectable by either ppVD or cpRNFLT. 방법 : The ppVD, cpRNFLT, and VF sensitivity (VFS) were evaluated using spectral-domain optical coherence tomography (OCT), OCT angiography (OCTA), and standard automated perimetry, respectively. Correlations between ppVD and VFS (vascular-function) and between cpRNFLT and VFS (structure-function) were assessed using both linear and fractional-polynomial (FP) models. Davies’ test was used to determine the VFS below which progressive VF loss becomes not detectable (breakpoints) by ppVD or cpRNFLT. 결과 : Both ppVD and cpRNFLT had significant linear correlations with VFS globally. In superotemporal, temporal, and inferotemporal sectors, the correlations were better explained by FP model than by linear model for both ppVD (P≤.009) and cpRNFLT (P≤.008). Breakpoints for VFS in the vascular-function and structure-function correlations were -8.28 and -6.41dB in superotemporal sector, -11.83 and -8.42dB in temporal sector, and -7.92 and -6.28dB in inferotemporal sector, respectively. 결론 : Sectoral ppVD had curvilinear correlation with VFS, and had significant breakpoints below which VFS decline became not detectable. The VFS breakpoints were lower for ppVD than for cpRNFLT, indicating potentially larger dynamic range of ppVD to detect more advanced VF damage.
