Aqueous complement fragment C3a is the most reliable predictor of rejection in pig-to-rhesus corneal xenotransplantation

목적 : Corneal xenotransplantation can be an alternative to allotransplantation. We investigated predictive biomarkers for rejection in pig-to-rhesus corneal xenotransplantation which was performed in rhesus (n=43) with different immunosuppressants. 방법 : Rejection score (0-12) was calculated with opacity, edema, and vascularization. Score≥6 was defined as rejection. The number of T/B cell subsets, levels of anti-αGal IgG/M, donor-specific IgG/M from blood, and complement activation fragment (C3a) from plasma or aqueous humor (AH) were evaluated every two to four weeks. Multiple regression analysis and area under the receiver operating characteristic curve (AUROC) were used to evaluate predictive ability of the markers. 결과 : For predicting rejection until 3 and 6 months, AUROC of AH C3a at 4 weeks were 0.957 and 0.900, respectively (all P<0.001) and AH C3a of 9.85 and 12.35 ng/mL were the best cut-off values (sensitivity: 100% and 66.7%; specificity: 86.7% and 100%, respectively). In multivariate analysis, significant predictors of rejection scores at the same time, and after 2 and 4 weeks were AH C3a (β=0.662, 0.647, and 0.613, respectively, all P<0.001), plasma C3a (β=0.100, 0.124, and 0.131, respectively, all P≤0.022), and anti-αGal IgG (β=0.287, 0.273, and 0.241, respectively, all P<0.001). 결론 : Our results suggest that AH C3a is the most reliable predictor for rejection in pig-to-rhesus corneal xenotransplantation, followed by plasma anti-αGal IgG and C3a.
