Diabetic choroidopathy: choroidal vascular area and density in diabetic eyes using UWF ICG angiography

목적 : To compare choroidal vascular area and density in diabetic eyes and controls, using ultra-widefield (UWF) indocyanine green angiography (ICGA). 방법 : Eighty-nine diabetic eyes and 12 control eyes were imaged using UWF fluorescein angiography (FA) and ICGA. The patients were classified according to the severity of diabetic retinopathy (DR). All UWF images were stereographically projected and binarized for the quantification of vascular areas. The choroidal vascular areas were calculated by subtracting vascular areas of FA images from those of ICGA images. The calculated choroidal vascular area and density were compared among groups from four different areas (<3 mm, 3~10 mm, 10~15 mm, and >15 mm zones), and correlated with severity of DR. 결과 : The choroidal vascular areas and density in total area were 190.83 ± 13.42 mm2 (27.10 ± 3.30 %) in control, 192.69 ± 16.24 mm2 (27.11 ± 1.77 %) in no DR, 192.25 ± 19.42 mm2 (26.92 ± 2.02 %) in mild to moderate nonproliferative DR (NPDR), 186.71 ± 20.83 mm2 (25.97 ± 2.07 %) in severe NPDR, and 177.23 ± 18.67 mm2 (24.66 ± 2.32 %) in proliferative DR (PDR). The DM severity was correlated with the choroidal vascular density (P=0.003, rho=-0.300 in total area zone; P=0.025, rho=-0.227 in <3 mm zone; P=0.013, rho=-0.220 in 3~10 mm zone; P=0.019, rho=-0.239 in 10~15 mm zone). 결론 : Choroidal vessel density and area can be extracted from subtraction and binarization of projected UWF FA and ICGA images. The choroidal vascular density is significantly reduced in more advanced stages of DR, especially in PDR. This suggests the possibility that diabetic choroidopathy may occur simultaneously with or as a result of diabetic retinopathy.
