Inner Retinal Thickness and Retinal Vessel Changes after Vitrectomy in Macula-off Retinal Detachment using Swept-Source Optical Coherence Tomography Angiography

목적 : To compare post-vitrectomy inner retinal layer thickness and retinal vessel density (VD) between eyes with macula-off and macula-on rhegmatogenous retinal detachment (RRD) using swept-source optical coherence tomography (SS-OCT) and OCT angiography (OCTA) and to identify OCTA factors associated with visual outcome. 방법 : We retrospectively reviewed 50 eyes that underwent pars plana vitrectomy with primary RRD. Eyes with macula-off (off-RD) and macula-on RRD (on-RD) were compared with healthy fellow eyes. Both OCT and OCTA were performed at 6 months after surgery and off-RD group was divided into two subgroups according to the presence of outer retinal defect (defect group and intact group). The correlation between postoperative best corrected visual acuity (BCVA) at 6 months and SS-OCT and OCTA measurements were analyzed. 결과 : 20 eyes with off-RD and 10 eyes with on-RD were included. In the off-RD group, the total retinal layer thickness was significant decreased compared to the fellow eyes (215.6±21.7um vs 244.9±30.9um, p=0.004). Inner retinal thickness and retinal capillary plexus VD did not show significant difference. In the defect group, choroidal capillary plexus (CCP) VD was significantly decreased compared to the fellow eyes (p=0.028), but not in the intact group. The postoperative BCVA at 6 months was significantly correlated with CCP VD ratio (RRD eye / fellow eye) in the defect group (R=-0.535,p=0.024). 결론 : The CCP VD may be related to anatomical restoration of the outer retinal layer in macula-off RD. CCP VD on OCTA may be one of OCTA marker to predict visual outcome after surgery of macular-off RD cases.
