혈관빛간섭단층촬영을 이용한 신생혈관 패턴에 따른 연령관련 황반변성 재발의 인자

목적 : To study the features of type 1 choroidal neovascular membranes (CNV) in age-related macular degeneration (AMD) using optical coherence tomography angiography (OCTA) in patients with recurred CNV before and after recurrence. 방법 : OCTA images were obtained using RTVue XR Avanti with AngioVue. The CNV lesion was divided into an inner segment (inner 50% of the lesion) and outer segment (outer 50% of the lesion) to quantitatively analyze the differences and evaluate the location of greatest activity. The CNV lesions were also graded as having an overall immature (tangle of small, uniform, branching capillaries), mature (large dilated vessels in a medusa or seafan pattern, or hypermature (dead tree appearance) vascular pattern. 결과 : 26 eyes of 25 patients were included. 7 eyes (23%) showed immature, 13 eyes (43%) showed mature, and 6 eyes (20%) showed hypermature pattern of CNV. When a recurrence occurred, CNV density was significantly increased in both inner and outer segment of the lesion (p=0.018 for inner; p=0.043 for outer segment) in immature pattern group, while CNV density increased only in outer segment of the lesion in mature pattern group (p=0.064 for inner; p=0.023 for outer segment). Hypermature CNV group showed no significant CNV density changes. (p=0.465 for inner; p=0.249 for outer segment) 결론 : OCTA is a new, noninvasive imaging modality that can be used to perform quantitative and qualitative analyses of CNV lesions. OCTA may provide biomarkers of activity and guide the evaluation and treatment and monitoring of CNV in AMD.
