목적 : To analyze the refractive outcome of two hydrophobic acrylic intraocular lenses (IOLs) and to present a computerized calculation scheme for optimization of IOL constants based on the refractive results of a surgeon
방법 : We retrospectively reviewed the medical records of patients who underwent cataract surgery and implanted extended depth of focus (EDOF) IOL (TECNIS Symfony ZXR00, n=24) or +3.25 Diopter add (D) diffractive bifocal IOL (TECNIS ZLB00, n=25). Evaluations were performed at least 1 month following cataract surgery, including measurement of visual acuity, refractive error and defocus curve testing. The preoperative biometrical data were assessed with the refractive outcomes in order to analyze the deviation from target and to optimize SRK II, SRK/T, Hoffer Q, and Haigis formulae.
결과 : UCVA, LogMAR of EDOF IOL group was 0.11±0.07 and +3.25D IOL group was 0.12±0.0. Spherical equivalent of EDOF IOL group was -0.96±0.40 and those of +3.25D IOL group was -0.43±0.41. Based on this data, it could be demonstrated that using the A constants provided by the manufacturer, both IOLs, especially EDOF IOL, showed myopic shift. For the EDOF IOL/+3.25D IOL constants of : A=118.4/119.6(SRK II), A=118.3/119.3 (SRK/T), and pACD = 5.21/5.78 (Hoffer Q) and lenses constant triplet for Haigis formula of a0=-1.776, a1=0.191, a2=0.248/a0=-1.220, a1=0.191, a2=0.248 were derived after optimizing.
결론 : Ophthalmic surgeons are encouraged to check refractive outcomes after cataract surgery in terms of quality control. Especially, in the case of multifocal IOL, it is possible to maximize visual performance by reducing systematic deviation of refractive outcome through optimization.
