TREATMENT OF SEVERE BILATERAL OCULAR SURFACE DISORDERS BY CULTURED ORAL MUCOSAL EPITHELIAL TRANSPLANTATION

Purpose : Bilateral severe ocular surface disorders by total limbal stem cells deficiency is a challenge to ophthalmologists. Ocular surface reconstruction by autologous cultured oral mucosal epithelial transplantation is a new solution that has been successfully done in the developed countries. We evaluated the results of cultured oral mucosal epithelial transplantation in rabbits and applied that in the patients with severe bilateral ocular surface disorders. Methods : Prospective experimental and clinical study. The cultured oral mucosal epithelial sheets were transplanted in 15 experimental rabbit’s eyes and 20 patient’s eyes with bilateral ocular surface disorder Results : In the experimental phase, 14 rabbit eyes achieved good results, one eye with moderate results. In the clinical phase, 14 cases had improved vision, stable ocular surface, inflammatory reduction and decreased corneal neovascularization; 6 cases failed, in which 2 cases had graft melting because of chemical substance penetrated into the anterior chamber and 4 cases had recurrent corneal neovascularisation. Conclusion : Cultured oral mucosal epithelial transplantation is a relatively safe and effective procedure to treat patients with severe bilateral ocular surface disorders.
