본문 : The Heimann-Bielshowsky phenomenon (HBP) is a unilateral vertical nystagmus, characterized by coarse, slow pendular movements, which typically occurs in an eye with profound visual loss. It is very underrecognised condition. Hypotropic DVD is also associated with monocular visual deficit and characterized by a dissociated hypodeviation of one eye that develops intermittently only while the other eye is fixating a target. However, a case of hypotropic DVD with HBP has never been reported in the literature. Here, we report a case of a hypotric DVD with HBM. A 58-old female presented with unilateral vertical nystagmoid movement in her left eye. She was ortho at near but had hypotropia of 20-25 PD at distance. Previously 10 years ago, she had undergone R&R in her left eye for her sensory esotropia of her left eye, had been orthophoric for few years. She was initially diagnosed with hypotropic DVD and underwent inferior rectus 7 mm recession of her left eye. After strabismus surgery, she was ortho at near and distance without hypotropic DVD, however, her eye was still moving vertically above horizontal axis. Considering her ocular manifestations prior to and after strabismus surgery, she seems to have the characteristics of both hypotropic DVD and HBP. |
