Outcomes of Direct cyclopexy for traumatic cylcodialysis cleft

Purpose : Cyclodialysis cleft is a common cause of persisting ocular hypotony and vision loss following trauma. Our purpose was to evaluate visual outcome,intraocular pressure control following direct refixation of ciliary body to the scleral spur. Methods : Retrospective case series of 7 eyes of 7 patients, aged (41.6±15.31yrs), presenting between 2006-2016. Results : All patients had a single cleft, extending (2.14±1.21)clock hours. Preoperatively,all eyes showed hypotony(1.71±1.25mmhg) with disc and macular edema. Median duration of hypotony prior to surgery was 28 weeks. Median postoperative followup period was 24 weeks. Intraocular pressure (13.14±10.41mmhg) improved significantly(p-0.005) and cleft closure was seen on gonioscopy / Ultrasound biomicroscopy in 6 eyes. There were no serious complications. 1 patient had persisting glaucoma at 34 weeks, well controlled on medication . 5 patients had postop visual acuity > 6/18 on Snellen’s chart. Compromised acuity was due to cataract and posterior segment trauma sequelae. Conclusion : Direct cyclopexy is a successful and relatively safe option for moderate sized clefts (2-5 clock hours).
