급성 산재성 뇌척수염에 의한 양안 시신경염 및 가성근시 1례

본문 : Acute disseminated encephalomyelitis (ADEM) is an acute or subacute inflammatory demyelinating disorder of the central nervous system that occurs most often in children. Neurologic signs such as behavioral change or alteration in consciousness typically appear days to weeks following viral infection or vaccination. Optic neuritis is a well-known ocular manifestation of ADEM, and it is the presenting sign in a quarter of childhood cases of ADEM. However, pseudomyopia has never been reported as one of ophthalmologic manifestations of ADEM. Here we report a case of a 5- year old child who initially diagnosed with ADEM and was treated by intravenous high-dose steroid therapy. After few days, she presented bilateral vision loss due to bilateral optic neuritis and pseudomyopia. Her vision improved from FC to 20/40 after steroid therapy but still had pseudomyopia. We prescribed atropine and her vision improved to 20/25. We should consider not only bilateral optic neuritis but also pseudomyopia when evaluating ADEM patients with bilateral vision loss.
