백내장 수술 환자에서 개검기 착용이 안압에 미치는 영향

목적 : To assess the effect of an eyelid speculum on the intraocular pressure (IOP) in patients undergoing cataract surgery. 방법 : Intraocular pressure (IOP) was measured using Tono-Pen AVIA (Reichert, NY, USA) before and after removal of eyelid speculum at the end of cataract surgery. Measured 10 IOP values were averaged. Change in IOP was calculated by subtracting mean values before and after speculum removal. Two types of speculums were used: Liebermann speculum in 104 eyes and Krats-Barraquer wire speculum in 31 eyes. Independent t-test was conducted to compare mean IOP values and change in IOP between 2 speculum groups. 결과 : The mean IOP with and without Liebermann eye speculum was 16.27±3.82 mmHg and 12.29±3.69 mmHg, respectively (p<0.05). The mean IOP with and without Krats-Barraquer wire speculum was 15.09±4.85 mmHg and 13.64±4.15 mmHg, respectively (p<0.05). The mean IOP change was 3.98±2.78 mmHg in Liebermann speculum group and 1.45±1.57 mmHg in Krats-Barraquer wire speculum. There was a statistically significant difference in two speculum groups (p<0.05). 결론 : Krats-Barraquer wire speculum caused less change in IOP after removal compared to the Liebermann eye speculum. Maintaining appropriate IOP after surgery is important for wound self-sealing. Cataract surgeons should take speculum effect into consideration when adjusting IOP.
