스마일수술 후 발생한 중심성 독성 각막병증 1예

본문 : A 35-year-old man presented with a 2-month history of decreased vision in the right eye. He had undergone small-incision lenticule extraction (SMILE) surgery 7 months earlier. Preoperatively, his central corneal thickness was 565 μm. Seven months postoperatively, examination revealed the typical central toxic keratopathy triad of corneal thinning, hyperopic shift, and a reduction in best-corrected visual acuity to 20/50. Manifest refraction in his right eye was +4.00 sph -0.75 cyl × 90 and the central corneal thickness was 373 μm. SMILE has been considered as an alternative to conventional laser in-situ keratomileusis because of its benefits of reduced denervation, less risk of dry eye postoperatively, and no flap-related complications. However, SMILE may cause central toxic keratopathy even though the incision required is small. Given the increasing use of SMILE, we believe that all refractive surgeons should be aware of this rare complication
