MACULAR STRUCTURE AND RETINAL NERVE FIBRE LAYER ABNORMALITIES IN THALASSEMIC INDIVIDUALS BY SPECTRAL-DOMAIN OPTICAL COHERENCE TOMOGRAPHY IN SABAH, EAST MALAYSIA

Purpose : To evaluate macular and peripapillary retinal nerve fibre layer (RNFL) thickness in paediatric and adult patients with Thalassemia major (tha-major) in comparison with healthy controls. Methods : This is a case-controlled cross sectional study. Patients fulfilling diagnostic criteria for tha-major (n=99) and healthy age- and race-matched controls (n=99) were enrolled into the study which consisted 31 paediatric patients (13 – 18 years) and 68 adults (18 – 49 years) in each group. Cirrus Spectral Domain Optical Coherence Tomography was used to measure macular structure and peripapillary RNFL thickness. Diffferences between tha-major patients and healthy subjects were assessed and recorded for statistical analysis. Results : The mean central foveal thickness for the paediatric tha-major group (227.42 ± 21.36 µm) and adult group ( 229.63 ± 23.53 µm) were significantly lower compared to the paediatric control group (240.42 ± 20.03 µm) and adult group (241.87 ± 19.50 µm ), with a p value of 0.016 and <0.001 respectively. Macula cube volume and macula average cube thickness showed no differences between the two groups. Mean average RNFL thickness in the paediatric tha-major group (99.68 ± 14.27µm) was lower compared to the control group (102.65 ± 7.03µm), however not statistically significant. Positive correlation was found between mean RNFL thickness with ferritin level in tha-major patients (p = 0.033). No correlation was found between the other disease parameters such as hemoglobin level, duration of blood transfusion and other OCT parameters such as mean macular thickness and average RNFL thickness in tha-major patients. Conclusion : Central foveal thickness were significantly thinner in both paediatric and adult tha-major group compared to control group. The average RNFL thickness was similar in tha-major and control group. Positive correlation was found between mean average RNFL thickness with ferritin level in tha-major patients.
